EXPLAINER: Lab results and their interpretation

Hi @james1,

Welcome to the forum and sorry you have been through the stress of uncertainty about your situation.

Indeed, the results you have posted are consistent with a false positive for HBsAg. It is worth getting other tests to confirm this, including an anti-HBc test (to test for previous exposure).

It may also be worth getting a booster for HBV, so that you have the peace of mind that you are protected in cases of future exposure.

Hope this helps,
Thomas

Hi @Bansah1

Thank you so much for the perspective. Yes, it most certainly helps.

And will get vaccinated again ASAP.

Will discuss with my primary if he requires any more HBsAG before I proceed to vaccination.

Based on last two negative HBsAG, the first positive notwithstanding, I will proceed as though I’m negative.

Would assume the first was a false positive? Or would you recommend I consult with a specialist?

Thank you so much for the advise!!!

@ThomasTu ,

Thank you so much, Thomas.

anti-Hbc also came back negative.

Hi @James1,
You are welcome. Yes, your assumption is correct. At this point with 2 negative tests, I think it is more likely the first was a false positive. Get the vaccine now, and then after 2-3 months check anti-HBs. If it is positive then it confirms the negative test was correct. Best, Bansah1

I agree with @Bansah1 that the first one was likely a false positive, particularly given your anti-HBc is negative also.

@ThomasTu @Bansah1,
Very much appreciate your feedback and am very grateful for your respective perspectives.

How common are HBsAg false positives?
Controlling for recent vaccination, ofc.

Have been going over different threads here and did some research online and seemingly very little information on the subject.

Hi @James1,

The false positive rate will be dependent on the specific test used, and there are several kits used globally to measure HBsAg. The false positive rates can be found with some digging (in the kit paperwork or in the studies needed to be carried out for appropriate approval for use in the clinic).

Sorry, I can’t be more specific without more information.

Cheers,
Thomas

I can’t give you a number, but such test results do happen sometimes. There have been some among patients on this platform. It does happen. Best, Bansah1.

You had these tests results before you began the vaccination process so I don’t see any correlation here. Two separate events without any influence on either one.

Hi everyone,

I hope you all are doing well. I have some concerns/questions regarding my husband’s test results. He has been on Vemlidy for about 3 months (38M).

Red blood cells: 6.02 (high)

Hemoglobin: 18.2 (high)

Hematocrit: 53.8 (high)

Ferritin: 208 (range 30-400)

Iron: 152 (range 38-169)

Iron saturation: 52% (range 15-55%)

Alkaline Phosphatase: 146 (high)

AST: 44 (high)

ALT: 52 (high)

HBV quantity <10 IU/mL

I am concerned about iron overload even though his results do not indicate that yet. Also, his liver enzymes are still high. Is iron overload dangerous? What should we do to improve those? Thank you all so much. As always, I am very grateful I have found this group. Looking forward to hearing your replies!

Hi @Tran-ng,
I am not sure there is anything you have to do about his iron since they are in the normal range. Too much iron can cause some problems, but your husband does not have this problem based on his results. I won’t worry about this. His AST/ALT are slightly elevated, but should not be a huge concern. It should be a concern if it remains high for some time. Fluctuations are common from time to time, so you should expect that. He has been on treatment for 3 months now, so give the medicine time to work. It will take some time before you notice its effect. Don’t get too alarmed by this. Best, Bansah1

Hi I have a question please the first time I got information about my status was in Feb 2023 and my GP just told me as simple Inactive HBV seen you are not effectious and do not need treatment that was it

I do not know what test was since I was new to this History I just went without taking his words as just a normal infection that was gone.

A year later I was going into a relationship so I decided to screen and that lad did Hbsag and stated not reactive on two test I even donated blood to the blood Bank I year on I was not in good shape issues with blood pressure and urine color so I saw a GP and she did my blood culture and organs and it came out that impression of a fatty liver

I was not ok with that fatty liver issues so I stopped eating for over two months to see it that will get rid of the fatty liver

Then it came to my mind again to check on the HepB my first GP spoke about as that was an inactive HBV so I asked for a HBV panel test since and the results 24 Dec.2024

HbsAg -
Hbcab+
Hbsab immune
Hbsab Value 76.10 UI/Ml
HbeAg-
HbeAb+
And Two liver test readings were all in range on the first and six months after the the second had suger on 6 and the Triglycerides 3.12

I have been very worried I have had two kids 16 and 9 years

It is my worries this very GP who told me of the infection mad a comment during my wife’s pregnancy lab "oh the is a list infection that is common with cat and I asked was that going to affect the baby he said no I am suspicious of that comment and if that was the HBV why he did not act to protect my baby in 2015

So with this result what is my situation. A New GP says am fine and healhty . But after following the HBV Seminar on youtube I need some help to understand my situation
From you or Thomas

Thanks you

Hi @Tonyrog,
Thanks for sharing your results and sorry for the confusion. Based on the results you shared from December 24th, you do not have hepatitis B as indicated by HBsAg negative. Plus, you do have immunity with a high antibody as indicated by HBsAb and a value of 76.10 UI/mL. These are great because you do not have to worry about this virus.

With your first GP, I think it is possible the results were a false positive or it was accurate in showing an acute infection meaning you were infected in the past but have cleared it as indicated by HBcAb positive result. We will never know, but the good news is you don’t have HBV, and you have a strong immunity against it.

Regarding the fatty liver issue, please work with your doctor to understand more about the source. Hepatitis B can be ruled out. What else might be causing it needs further investigation.
I hope this is helpful. Best, Bansah1
