In Australia, we administer the Hep B vaccines to newborns within 24 hours of birth to prevent HBV transmission.
Reference: Infants born to mothers who are hepatitis B surface antigen–positive are recommended to receive both hepatitis B vaccine and HBIG

I’m guessing this is your first child and you want the best for your family. You can discuss this with the mother’s healthcare provider to ensure the best outcome.

Currently pregnant with my second child, first found out that my first child was positive for HBsAg and had been vaccinated with HBGB until 9 months, thank God, when my first child was tested, she was 9 months old, it was negative… and now pregnant with my second child, the doctor suggested an HBV DNA test and the results are above… The doctor will administer medication at 28 weeks, and I was surprised that the doctor also suggested a fibro scan on August 27… Was my doctor’s action correct?

I understand your concern about the use of Fibroscan during pregnancy.It’s ultrasound based techniques for diagnostic assessment of liver injury in patients with chronic liver diseases. It’s no longer contraindicated to use in pregnant women.I think that your doctor have the reasons and correctly decision making.According to WHO guidelines for CHB treatment, Fibroscan is used to assess the fibrotic staging in order to consider continuous treatment in pregnant women regardless of HBV DNA or ALT levels.The indications for who to treat pregnant women with CHB as follows below. I hope this may help you .

Thanks for asking your question. With such a high viral load, guidelines would recommend that your wife be put on antiviral treatment to lower the risk of transmission to the baby (as well as birth dose vaccination and possibly HBIG).

If HBV DNA and HBEAG are active, will it definitely cause fibrosis or cirrhosis? I’m very worried about my wife’s condition in the future. now my wife doesn’t have any symptoms… thank you

Hi @Helmy,

No, fibrosis and cirrhosis are not inevitable. Antiviral medications will reduce the progression of liver disease.

I want to tell you about my experience, my wife had reactive hbeag and hbv dna was detected 1,07x108(log 8). initial control after the test is completed the doctor plans to administer the drug at the age of 28 gestational age… and now that I’m 28 weeks old, I went back to the public hospital in my area and it turned out that the doctor had changed, not the previous one. In that public hospital, there are lots of internal medicine doctors taking turns. The doctor who previously planned therapy for 28 weeks of gestation stopped after birth until breastfeeding was finished, but the one who served me today prescribed medication for life without stopping, which really confused me. and what’s more, I’m not sure the current doctor doesn’t seem to understand what to do considering the doctor called more than 2 people asking about how my treatment was going without asking about my history and just jumping to conclusions… Is tdf safe for breastfeeding mothers? I’m worried about the wrong care and risk to my baby

That might have been frustrating witnessing that. But you should not have any issues breastfeeding your baby. There are patients on here who have breastfeed their babies and had no issues. I hope some of them could share their experience with you. If it will make you more comfortable try seeking a second opinion from a provider that is experienced in treating HBV but more so in pregnant women. Best, Bansah1
